In 2007 the Institute for Healthcare Improvement (IHI) introduced the Triple Aim, a new framework for care delivery with three distinct goals:

  1. Improving the patient experience (quality, access and reliability of care)
  2. Improving the health of the population
  3. Controlling or reducing the total cost of care

Following implementation of our shared electronic health record (EHR), all CapitalCare clinicians involved in a patient’s care have immediate access to medical information enabling them to address individual patient needs in an effective and cost-efficient manner. We are using our information systems to identify those patients who are at highest risk for complications of their chronic health conditions and offering them the services of care coordinators. These registered nurses have the experience and training to focus on complex healthcare needs and help patients access the services they need. They work closely with other team members, including clinical pharmacists and—when needed—certified diabetes educators. As we continue to expand our care teams, we will be adding the services of clinical and behavioral social workers.
