CapitalCare provides comprehensive benefit packages* and continually reviews and/or enhances these benefits in order to meet our employee’s and/or family’s needs. These benefit options offered provide protection for employees and their families, along with helping to build financial security for the future.
Our descriptions below provide the highlights of some of our benefits and are not all-inclusive. Because benefit packages can change, these descriptions are subject to change without notice. There are also other benefit programs and other voluntary benefits offered by CapitalCare not listed, as well as many discounted services from vendors offered to all employees.
CapitalCare offers competitive healthcare insurance for you and your family. We offer four CDPHP plans which include three tiers: Individual, Individual +1 and Family coverage. Our Plans include Health Savings Accounts (HSA) and Health Reimbursement Accounts (HRA). The plans provide hospitalization, medical and surgical services, outpatient services, well care for children and adults, a prescription drug plan with mail order option, behavioral health coverage, diagnostic services and a personal health advocacy program. There are no "pre-existing" clauses in our plans. Coverage begins on the first of the month following date of hire.
CapitalCare further enhances your healthcare coverage by providing a dental care benefit program administered by Delta Dental of New York. This plan provides dental services from oral examinations and cleanings to major services including oral surgery and prosthetics. Coverage can begin on the first of the month following date of hire.
Flexible Spending Account
CapitalCare understands that benefits are an important part of your total compensation. Under Section 125 of the Internal Revenue Code, employers may allow employees to pay for certain types of medical, dental and dependent care expenses with pre-tax dollars. Under the Flexible Spending Account arrangement, you can reduce your current taxable income and also increase your spendable income each pay period.
CapitalCare’s vision benefit offered is EyeMed Vision Care’s Network which consists of private practicing optometrists, ophthalmologists, opticians, and optical retailers such as LensCrafters®, Pearle Vision®, Sears Optical®, Target Optical® and JC Penney Optical® This plan covers exams, glasses, contacts and offers a variety of eye care discounts.
CapitalCare provides group term life insurance coverage paid for in full. In the event of an accidental injury, you are also covered for dismemberment benefits as outlined in the contract. Coverage is available at no cost to regular full-time and regular part-time employees. The amount of coverage is generally one times base salary.
CapitalCare maintains a 401(k) and profit sharing plan designed to provide you with a future benefit at retirement and portability of your benefits if you leave CapitalCare before retirement. The plan allows for personal, voluntary contributions AND a company match contribution after the eligibility period is met. You are immediately 100% vested in CapitalCare’s match contributions to the 401(k).
Voluntary Benefit Programs
CapitalCare provides you the opportunity to purchase additional voluntary benefits at a group discounted cost. These include but not limited to voluntary life, disability and cancer protection plan.
*Eligibility is determined on base hours.
